Basic poppet work. It would work better if you make it out of cloth. Specifically a piece of your targets clothing, but any cloth will do, and stuff it with herbs that will help in your goal. This seems too simplistic to do much.
Not only as Tadashi said, but the use of the moon water isn't the best idea. You know what happens when cloth or stuffing gets wet? It molds. It is probably best to leave the moon water out.
