Or are they all the same no matter what they look like and how they have been handled or are some better?

stones in genral can be any shape form even the natural way they are the same what does matter is the size and where you wear them some are better worn in diffrent place on the body

So if one is rough and sorta old/gross looking (Kinda like an ice cube looking) then it would be just as good as the perfect looking one?

yes though most people liked the shaped ones bacause they tend to look more beautiful

stones and rocks and crystals are like humans, Not all of us are perfect. You can use any that appeal to you or that you like.
